Application Process

• Download and complete the application and return to Lakeside Township, PO Box 1012, Isle, MN 56342

• A check in the amount of $150.00 payable to Lakeside Township; must be made at the time of application

• Culvert must be installed correctly within 60 days of application approval. Final inspection and approval will be made by an authorized agent of Lakeside Township

• Put up flagging where the culvert is to be placed

• Lakeside Township will check location for correct size of culvert
